Abbreviations - If you are audited, don"t count on a chiropractor reviewing your records who"ll know what "PRL" or "ASRA" or even "Sublux" means. For that matter, even just a non-chiropractor may have problem with non specific abbreviations like "PT." Did you send the individual for Physical Therapy or part-time work? The auditor isn"t going to try to translate your abbreviations, especially if a key isn"t absent that describes their patient. An excellent SOAP note with one unidentifiable abbreviation can cause you to neglect your audit. Therefore, make sure that you"re spelling it out clearly (another advantage to EMR, push the button and also the word comes out!) or that you supply a key that clearly translates all your abbreviations.
