Abbreviations - If you are audited, do not count on a chiropractor reviewing your records who"ll know what "PR L" or "ASRA" or even "Sublux" means. The auditor is not going to try and interpret your abbreviations, particularly when a key isn"t absent that clarifies them. A great SOAP note with one abbreviation that is unidentifiable can make you neglect your audit. So, make sure you are spelling it out clearly (another advantage to EMR, push the button and the word comes out!) or that you provide a key that clearly translates all your abbreviations.

It"s definitely common practice when my chiropractic clinic is entered by a person that is affected to do a thorough history, assessment and then, if significant, x rays.
